The North West Ageing Well Together is a cross sectoral partnership whose aim is to deliver a range of services, aimed at improving the health and well being of vulnerable older people in the Derry City and Strabane District Council area.
The project combats the following key issues:
· Combating Isolation (including carers)
· Empowering Older People – Information and Awareness Raising
· Accessing Hard to Reach Individuals
· Partnership Collaboration & Coordination
· Accessible Transport
The Services provided within the Project include:
Good Morning: A telephone support service to assist older and vulnerable people to
remain in their own homes.
Befriending: Support to older people who have particular needs e.g. requiring someone to accompany them to a health appointment or just spending an hour keeping the person company.
Handyman: Assistance with essential safety and security measures e.g. installing locks.
Community Transport: An accessible form of transport provided by Easilink.
Advice and Information: Free, independent, impartial advice and information provided by the Citizens Advice Bureau.
Carers’ Support: Support and a listening ear for those in a carer’s role.
